The Healthy Living Prescription is a tool for local Health Professionals to refer patients to the Healthy Living Centre for support from any of the centre’s services.
What the service offers
Sarah works closely with GPs and other Health Professionals to make sure that people who are taking positive steps to improve their health and wellbeing can find the right service for their needs.
This referral scheme is unique to the city, as it is provided by a multi-agency team of health professionals who all work together in a very holistic way, therefore ensuring the client referred gets the most appropriate service for them.
The scheme has proved to be very successful and has had some very positive outcomes. GPs are in favour of the scheme and the numbers of clients they have referred has doubled over a one year period, with more than 250 being referred this year.
Sarah is able to collate all referrals and outcomes and share this with the GPs which shows them clearly how many clients they have referred, what they have referred them for, and the outcome for that client.
The scheme is being rolled out further across other areas of East Brighton including Woodingdean and Saltdean over the next year.
